We help you make
important decisions about the safety and well-being
of an elderly loved one. Our goal is to
provide you and your physician with practical
information on an individual's cognitive and
physical ability to adequately care for him/herself.
The assessment is completed by a Physical Therapist
and an Occupational Therapist with extensive
training and experiences with elders. Also
available are the resources of a pharmacist and
social worker.
