Special Needs Assistance

Encore offers a comprehensive range of services including 24-hour nursing care for post-hospitalization and post-surgery recovery, respite care, on-site physical therapy and many other health services to address your specific needs. Planning & Coordination

The staff at Encore includes credentialed and experienced Social Service Professionals and Registered Nurse Case Managers who work together to assist you, or a loved one, to plan and coordinate a personal plan of care. Listed below are just a few of the many services Encore offers. We encourage you to make an appointment to learn more.

• Review of living options and identification of medical, health care and special services which may be required.
• Nursing assessment and care plan.
• Coordination of care with patient's/resident's physician and pharmacy.
• Planning for a continuum of care when Medicare stops paying.
• Specialized diets by our own Certified Dietary Manager and Registered Dietician.
• Coordination of social services as needed.
• Coordination of services from various community agencies including participation in support groups and counseling.
• Assistance with various resident needs, including bathing, dressing and grooming.
• Hospice care by provider of patient's/resident's choice.

Ongoing Social Support Services

Encore Communities has experienced Social Service Professionals who are available to assist you with your needs throughout your stay in our facilities.  They will coordinate services with you, your family members, and various members of our Encore Communities health care team as needed. Listed below are some of the Social Support Services offered by Encore Communities.

• Assistance communicating with family and health care professionals.
• Scheduled care conferences including patient/resident, family and other staff care providers to establish and monitor progress.
• Support for patient/resident and family during times of grief, loss, change and crisis.
• Assistance with resolution of difficult decisions.
• Resident advocacy, assurance of rights.
• Intervention of social/emotional needs.
• Assistance in accessing community resources and finances.
• Comprehensive discharge planning, beginning on the date of admission at Northwoods Lodge. Progress is tracked throughout your stay to prepare for a smooth discharge experience.
•  Assistance in obtaining living arrangements and options after discharge, including independent living, assisted living, retirement centers, etc.
• Assistance in accessing home care services.
