Welcome to Encore
Communities Rehabilitation Center at Northwoods
Lodge. Our highly-credentialed staff provides
comprehensive physical, occupational and speech
therapies to residents of Encore's retirement and
assisted living communities and, on an outpatient
basis, to those living in the greater Kitsap, Mason
and Jefferson county areas.
Using intensive therapy
programs, we assist our patients to achieve their
maximum, functional ability as soon as it is safely
possible. Our goal is to help patients return
to their own environment and enjoy their
independence. The intensity of the individual
rehabilitation programs are based on specific
diagnosis, endurance, ability, and physician's
prescription.

Our
Safe at
Home Functional Assessment Clinic offers
practical information on an individual's
cognitive and physical ability to adequately
care for him/herself. The assessment
is completed by a Physical Therapist and an
Occupational Therapist with extensive
training and experience with elders.

pharmacist and social worker
